     Intake Form, new Client questionnaire in Adobe pdf format - use this form to itemize your debts.  I prefer that your questionnaire is mailed, e-mailed or faxed to me before your meeting, but if you cannot, simply bring it with you an appointment. Intake Form, new Client questionnaire in Word Format - if you don’t want to bother with Adobe pdf and you are using Microsoft Internet Explorer as your browser, you can download this intake form into Microsoft Word and print it out.
